Shoulder
Biceps Tendinopathy
Also called: Bicipital tendinitis
Irritation of the long head of the biceps where it runs through its groove at the front of the shoulder — a frequent companion of cuff problems and overhead loading. Produces front-of-shoulder pain that follows curling, pulling, and reaching.
Common signs
- Front-of-shoulder pain, tender directly in the groove
- Pain with curls, chin-ups, reaching, or lifting away from the body
- An ache tracking down the front of the upper arm
- Occasionally clicking as the tendon tracks
Seek help now if
- A sudden pop with a new bulge in the arm ('Popeye' sign — long-head rupture; often managed conservatively but get assessed)
- Fever with swelling
- Progressive weakness beyond pain inhibition
If any of these is sudden or severe, call 911 or go to an emergency room. For the others, get examined before you start self-care.
What to do first
- Examine the neighborhood: isolated biceps tendinopathy is uncommon — cuff and labrum problems travel with it
- Modify the load pattern: reduce provocative curling/pulling volume, adjust angles
- Begin graded loading of the biceps and cuff together
The longer road
- A progressive program over 8–12 weeks with attention to shoulder mechanics resolves most
- Persistent groove pain despite quality care: re-image the shoulder for companion pathology
- The 'Popeye' rupture paradox: it often ends the pain and costs little function in non-athletes
Natural healing options we use
- Graded biceps and cuff loading program
- Manual therapy for the groove region, pec minor, cuff
- Chiropractic care for shoulder-girdle mechanics
- Attended e-stim; collagen and omega-3 support
About imaging
Clinical; ultrasound sees the tendon well when confirmation or companion-pathology mapping is needed.
Daily self-care
- Adjust grip and angle on pulls/curls during recovery
- Daily pain-free loading doses
- Posture work — a slumped shoulder narrows the tendon's real estate
