Achilles Tendinopathy
Also called: Achilles tendinitis
Overload of the body's largest tendon — midportion (a few centimeters above the heel) or at the heel insertion — from training spikes, hills, or years of load outpacing capacity. The treatment is famous and proven: progressive loading. Rest alone reliably fails.
Common signs
- Morning stiffness and pain in the tendon with first steps
- Pain that eases as you warm up, then is worse after
- Tender thickened area of the tendon (midportion type)
- Pain at the back of the heel with shoe pressure (insertional type)
Seek help now if
- A sudden pop with immediate weakness — rupture (see Achilles Tendon Rupture)
- Fever or a hot red tendon region
- History of fluoroquinolone antibiotics with new tendon pain (elevated rupture risk — get assessed)
If any of these is sudden or severe, call 911 or go to an emergency room. For the others, get examined before you start self-care.
What to do first
- Identify the type — insertional versus midportion changes the exercise details (insertional avoids deep heel drops early)
- Begin loading: isometric calf holds, then slow heavy calf raises — the core prescription
- Manage load: trim hills and speed work first; a small heel lift can calm insertional cases
The longer road
- Commit to a 12-week progressive program — tendon remodeling runs on tendon time, not wish time
- Runners return through a graded plan once daily-life pain quiets
- Stubborn cases: re-examine load, mechanics, footwear, metabolic factors (tendons dislike high blood sugar), and consider imaging
Natural healing options we use
- Structured tendon-loading program (the treatment)
- Manual therapy for calf complex
- Chiropractic care for foot/ankle mechanics
- Collagen + vitamin C pre-loading; omega-3s; attended e-stim for symptom relief
About imaging
Clinical; ultrasound/MRI for stubborn cases or rupture concern.
Daily self-care
- Daily calf loading as prescribed — consistency is the medicine
- Morning pain score tracks progress
- Avoid sudden barefoot/minimalist experiments mid-recovery
