Elbow, Wrist & Hand
De Quervain's Tenosynovitis
Also called: New parent's wrist, texting thumb
Inflammation of the tunnel sheathing two thumb tendons at the wrist's thumb side — the signature injury of new parents (repetitive baby-lifting), phone use, and grip-twist work. Sharp pain with thumb use and wrist tilting; very treatable with load modification and progressive care.
Common signs
- Pain at the thumb side of the wrist, sometimes swelling
- Sharp with gripping, lifting (especially the baby-under-armpits lift), jar-opening
- Positive stretch test: thumb in fist, tilt wrist down → reproduces the pain
- Occasional creaking sensation over the tendons
Seek help now if
- Trauma with snuffbox-area bone tenderness (scaphoid fracture — different urgency)
- Hot red swelling with fever
- Numbness (think nerve, not tendon)
If any of these is sudden or severe, call 911 or go to an emergency room. For the others, get examined before you start self-care.
What to do first
- Change the mechanics of the repeated lift: scoop with palms up, elbows in, wrists neutral (parents: this is the whole game)
- A thumb-spica brace for the heavy-use hours calms it while habits change
- Ice after provocation; begin gentle range work
The longer road
- Progress to graded loading of the thumb and wrist once sharp pain settles — capacity prevents recurrence
- Persistent cases despite genuine modification: injection is notably effective for this condition specifically; conservative care continues around it
- Address the load that isn't going away (the baby grows heavier) with technique and strength together
Natural healing options we use
- Activity-mechanics retraining (the core fix)
- Bracing strategy for high-load hours
- Manual therapy for the thumb-side tendon tunnel and forearm; graded loading program
- Attended e-stim; anti-inflammatory nutrition
About imaging
Clinical; ultrasound if confirmation needed.
Daily self-care
- Palms-up scoop lifting as the new default
- Brace during the heavy-use blocks, off for exercises
- Daily gentle glides progressing to strengthening
