Low Back & Pelvis
Piriformis Syndrome / Deep Gluteal Pain
Also called: Buttock nerve entrapment
Buttock pain, sometimes with sciatic-type leg symptoms, from irritation of the sciatic nerve by the piriformis and deep hip rotator muscles. A real but overdiagnosed condition — lumbar disc problems must be excluded first, because the treatments differ.
Common signs
- Deep buttock ache, worse with sitting (especially on hard surfaces or a wallet)
- Tenderness deep in the middle of the buttock
- Symptoms with hip movements, climbing stairs, or long drives
- Leg tingling that starts in the buttock, not the back
Seek help now if
- True progressive neurological deficits
- Symptoms clearly provoked by spine movements (points to lumbar origin)
- Night pain with systemic symptoms
If any of these is sudden or severe, call 911 or go to an emergency room. For the others, get examined before you start self-care.
What to do first
- Examination differentiates spine-driven from hip/gluteal-driven symptoms — this fork determines everything
- Reduce compression: cushioned sitting, wallet out of the back pocket, sitting breaks
- Begin gentle hip rotator stretching and glute activation
The longer road
- Chronic cases usually involve weak glutes forcing the deep rotators to overwork — strengthening is the fix, stretching is the relief
- Address gait, running form, and prolonged-sitting patterns
Natural healing options we use
- Manual therapy and instrument-assisted work for the deep rotators
- Chiropractic care for the lumbopelvic region
- Neural mobilization of the sciatic nerve
- Progressive glute strengthening
- Attended e-stim for the muscle guarding
About imaging
Used to exclude lumbar causes when the picture is unclear; the syndrome itself is a clinical diagnosis.
Daily self-care
- Figure-4 stretch and hip rotations daily
- Sitting breaks every 30–45 minutes
- Glute strengthening 3× weekly
