Whole-Body & Systemic
Polymyalgia Rheumatica
Also called: PMR
An inflammatory condition of adults over 50 causing profound aching and morning stiffness across both shoulders and hips — often arriving fast, sometimes 'overnight,' making dressing and rising from chairs suddenly difficult. Two reasons it matters: it's dramatically treatable once recognized, and it keeps company with a dangerous cousin (giant cell arteritis) that must be screened for.
Common signs
- Aching and severe stiffness across both shoulders and/or both hips
- Morning stiffness lasting an hour or far more; rolling over in bed is a project
- Rapid onset over days to weeks in someone 50+
- Fatigue, low mood, sometimes low-grade fever and weight change
- Trouble with hair-brushing, coat sleeves, rising from low seats
Seek help now if
- The cousin's signs — new headache (often temple), scalp tenderness, jaw fatigue when chewing, any vision change in someone with PMR symptoms — are a same-day emergency (giant cell arteritis threatens sight)
- Symptoms with prominent joint swelling of hands/feet (reconsider RA)
- Fever/weight loss dominating the picture (broader workup)
If any of these is sudden or severe, call 911 or go to an emergency room. For the others, get examined before you start self-care.
What to do first
- Get labs promptly: inflammatory markers (ESR/CRP) are typically markedly elevated — this diagnosis moves fast once suspected
- Screen out the cousin explicitly — headache/jaw/vision questions are part of every PMR conversation
- Co-management from the start: this condition's definitive treatment is medical, and response is often dramatic within days — natural care supports the terrain and the recovery
The longer road
- Through the treatment course (often a year or more of tapering): protect muscle and bone actively — resistance exercise and bone-nutrition strategy matter specifically because of the treatment used
- Gentle daily mobility keeps the stiffened regions moving as inflammation resolves
- Track morning stiffness duration as your barometer; report relapse signals early
Natural healing options we use
- Gentle mobility programming for shoulders and hips through recovery
- Progressive strength work to rebuild what inflammation and treatment cost
- Bone-protective nutrition (calcium, vitamin D/K2, protein) during the treatment course
- Anti-inflammatory eating pattern; sleep prioritization
- Manual care for comfort, adapted to disease activity
About imaging
Labs lead; ultrasound of shoulders/hips can support; imaging otherwise excludes mimics.
Daily self-care
- Know the cousin's signs cold and act same-day
- Move gently every morning as stiffness allows
- Strength train through recovery — muscle lost fast returns slowly
